Chronic Care

Internal Medicine Associates of Auburn implemented a Chronic Care Management program for their Medicare patients in March 2015.  The practice was one of the first to roll out their program this year.

The centers for Medicare & Medicaid identified the care of chronic health conditions as an important goal in January 2015. The program requires that the provider be the primary care physician for a patient with two or more chronic conditions, and needs to meet numerous requirements around using an electronic health record (EHR) for documentation, sharing continuity of care documents (CCD) with physicians and other clinician colleagues, and provide care coordination services to their patient every month.

Many could benefit from this program that Internal MedicineAssociates of Auburn is now offering to their patients.This program represents a critical step forward in recognizing the essential features of primary care including whole-person focus, and serving as the patients’  first contact for new health issues, and care coordination.

“Our goal is to offer high quality patient care coordination across the health care spectrum. We believe in treating the whole person, not just the disease. Our plan is to optimize our patient’s health, increase their quality of life and prevent hospitalization.”
