We hope you are having a wonderful start to your Summer.  July is almost here and we are excited to share the opening of our South Durham location to enhance access to our program.  You may receive additional notifications related to scheduled visits and announcements about our opening. Here are a few things we would like to share:
 
Who do I call?
Our entire administrative and nursing teams will continue to assist you for both locations.  We have additional team members on board to participate in your care.
 
What number do I call?
Our main office number remains 919-660-6746 and will be answered at both locations. Our answering service will continue to answer calls after business hours and you can contact your physician directly on Duke MyChart for medical needs.
 
Can I request to be seen at a specific site for scheduled visit?
Yes, Drs. Phelps, Paat, Wolf, Harpole and Waite will be practicing at both locations. 
Dr. Metz will practice at our South Durham location only.
 
Can I be seen by my physician for urgent visit?
 Yes, the location may vary depending on their scheduled site for the day.
 
Can I request a preferred site?
Yes, Dr. Metz will practice primarily at our new South Durham site.
Drs. Phelps, Paat, Wolf, Harpole, and Waite will have availability at Center for Living and South Durham.

From I-40 East:  Take Exit 276 Fayetteville Road. Turn left onto Fayetteville Road. Cross over I-40 and continue through the stoplight at Hwy 54.  Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign. 
From I-40 West:   Take Exit 276 Fayetteville Road. Turn right onto Fayetteville Road. Continue through the light at Hwy 54.  Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign. 
 
From Greensboro-Burlington on I-85 North:  Follow I-40 East and take Exit 276 Fayetteville Road. Turn left onto Fayetteville Road.  Continue through the light at Hwy 54.  Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign. 
 
From Henderson-Roxboro on I-85 South:  Take 15-501 Bypass to I-40 East. Take Exit 276 Fayetteville Road. Turn left onto Fayetteville Road.  Continue through the light at Hwy 54.  Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign. 
 
From Hwy 54 East:  At Fayetteville Road, turn left onto Fayetteville Road. Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign. 
 
From Hwy 54 West:  At Fayetteville Road, turn right onto Fayetteville Road. Turn right onto Crooked Creek Parkway.  Take the second right by the Duke Health sign.
