Duke Signature Care is excited to be the initial Duke Private Diagnostic Clinic (PDC) practice for our upcoming Joint Commission review for Primary Care Medical Home (PCMH) accreditation. This recognition supports our practice goals of providing patient centered, comprehensive, coordinated care for you. Our efforts to prepare for this review has been ongoing since last fall. Many of the recent developments that you may have noticed during your visits are geared to promote patient engagement, shared decision making and identification of patient priorities and health goals. We continue to strive to meet your access needs through Duke My Chart, our nurse triage lines and coming very soon, telehealth video visits.
We encourage you to speak to any of our staff and physicians with questions you may have and we invite you to continue to work with us to provide quality care in an unhurried fashion.
