Post-Discharge Antibiotics May Offer Limited Benefit After Pediatric Appendicitis

For children recovering from surgery for complicated acute appendicitis, a familiar safety step may offer less protection than many clinicians assume. A new systematic review and meta-analysis found that giving oral antibiotics after hospital discharge did not significantly reduce major postoperative complications, including intra-abdominal abscesses, organ-space infections (OSIs), or hospital readmissions. These findings raise important questions about the routine use of post-discharge antibiotics in pediatric surgical care and reinforce growing efforts toward antibiotic stewardship. By questioning whether "just-in-case" antibiotics improve recovery, the study may help hospitals refine discharge protocols, reduce unnecessary medication exposure, and focus follow-up care on children most likely to benefit from closer monitoring.
