Pediatric Stroke Burden Declines Overall but Rises Among Adolescents and in Low-SDI Regions

Pediatric stroke is rare but can cause lifelong disability, cognitive impairment and death, yet its long-term global pattern remains poorly characterized. This study assessed the burden of pediatric stroke--hemorrhagic stroke (HS) and ischemic stroke (IS)--among children and adolescents aged 0-19 years across 204 countries and territories from 1990 to 2021 using modeled estimates. It quantified prevalence, incidence, deaths and disability-adjusted life years (DALYs), and calculated estimated annual percentage changes (EAPCs) by age, sex and sociodemographic index (SDI). The analysis aimed to identify where progress has been made, where disparities are widening, and which age and stroke subtypes require targeted prevention, early recognition and health-system strengthening.
