“How comfortable are you filling out medical forms by yourself?”

Have you ever wondered why we ask that question during your visits here at Duke Signature Care?  What your answer means and how it affects your care?

The question itself does not have any right or wrong answer. We are simply looking for an honest response that correctly applies to you. This brief question is part of a learning assessment and allows for some quick insight about your comfort level with medical forms and medical jargon. At least once every year and sometimes more frequently, we will ask you this question during your visit. We encourage family members or caregivers to participate if they attend clinic visits with you or are routinely involved in your care.

Based on your response (Extremely, Quite a bit, Somewhat, A little bit, Not at all) the care team may adjust how your education is provided. For example, do you prefer hands on instruction or video instruction when learning how to inject insulin for the first time? Would a handout with illustrations of the heart help you understand a new diagnosis of atrial fibrillation? Should your family member or caregiver be included in the education if they pick up your prescriptions or dispense your medications?

With the increasing complexity of health information and emerging technologies available to us all, it is becoming more important than ever to ensure that we communicate clearly and efficiently as a team. Having a better understanding of your health literacy (the way you obtain, process, and understand health information) allows for improved communication, informed decision making, and quality health care.
